Need to make some lifestyle changes to lower your risk of heart disease and improve your overall health? Then we may have just what you have been looking for!

Our one-on-one mentored Lifestyle Management Program is based on a national program called INTER_xVENT. Offered through the Dupont Resource Center, this program can help you reduce your risk for heart attack or stroke and improve your overall health, fitness level and well-being by achieving and maintaining a proper weight, practicing good nutrition and exercising regularly and properly.

Participants are introduced to state-of-the-art, scientifically-based behavior modification techniques in the areas of nutrition, exercise, weight management, smoking cessation, stress, hypertension and diabetes management. To promote long-term success, the program stresses internal motivation, readiness, single-concept skill building, goal setting, problem solving, self monitoring, support groups and relapse prevention.

One-on-one mentoring includes two months of unlimited classes or two one-hour sessions with a certified personal trainer or certified yoga instructor at the Kachmann Mind Body Institute. One-on-one mentoring is available at two locations; at the Dupont Resource Center located on the Dupont Hospital campus or the Kachmann Mind Body Institute located on the Lutheran Hospital campus.

This program is unique in that it works in close collaboration with each participant's physician and is mentor-driven. Participants work closely with a trained healthcare professional who provides guidance and encouragement to help reach goals. The one-year program is customized to each participant's life and health situation and includes 24 one-on-one visits with a mentor, at which individualized goals and action plans are determined, educational materials and outcome reports are provided for the participant and his or her physician. Cost of the program is $550. All Lutheran Health Network employees are eligible for a 15% discount.
